Different Aspects of Pain Essay Pain is a subject to which all people can relate. There are many different types of pain, and people react to these pains in various ways. Pain is also caused from many different sources. It could be from grief, stress, or a significant event that occurs in one’s life. Pain is defined in the Dictionary as â€Å"mental or emotional suffering or torment.† The poetry of Robert Frost, James Langston Hughes, and Emily Dickinson all display different aspects of pain. Robert Lee Frost was born on March 26, 1874 in San Francisco, California where his father worked as a newspaper editor. This may have been where Robert was first exposed to the aspect of writing. Robert’s first published poem was in a school newspaper at the age of 16 where he wrote a poem on the subject of Cortez in Mexico. Although he attended Dartmouth for seven weeks and spent two years at Harvard, he never finished a college education with a degree. After he had gotten married, he worked as a schoolteacher, and during this period is when he spent time writing the majority of his poetry. After his teaching career, he moved to England to pursue getting his works published since his poetry was not accepted for publishing in America. His first two books of poems, A Boy’s Will and North of Boston, were published in England and then later in America due to the overwhelming popularity of them in England (Greenberg ix-x). Frost’s poem â€Å"Out, Out† tells a story of the tragic death of a boy due to a buzz saw. The title is an allusion to act five William Shakespeare’s Macbeth, where the main character, Macbeth, performs a soliloquy regarding the death of his wife: Out, out, brief candle! / Lifes but a walking shadow, a poor player / That struts and frets his hour upon the stage / And then is heard no more. It is a tale / Told by an idiot, full of sound and fury, / Signifying nothing. The allusion to Shakespeare in the title is appropriate to the subject matter because the soliloquy of Macbeth states that life is short, and inevitably will end. That is the message that Robert Frost is trying to convey in this poem. There are two different aspects of pain that appear in â€Å"Out, Out.† The first  one is the aspect of physical pain. This occurs when the buzz saw the boy is using, hits the boy’s hand and injures the hand severely. â€Å"As if to prove saws knew what supper meant, / Leaped out at the boy’s hand, / or seemed to leap (Frost 522)† The boy then begins to feel the pain of what has just happened, the physical pain of his hand being severed by the buzz saw. The next type of pain that can be seen here is the psychological pain, caused by stress. As a result of the boy’s injury, he begins to fall into pieces about the whole matter (clarify this somehow. â€Å"fall into pieces† sounds a little ambiguous as well as clichà ©) . The poem says that the boy â€Å"half in appeal, but as if to keep / the life from spilling. Then the boy saw all (Frost 522).† These two lines of the poem depict that the boy is old enough to understand what is going on with what is happening. His hand is injured beyond what the doctors can repair, and there is a high possibility of death because of what has just happened. The word ‘Life’ in this poem represents the blood that flowing from his hand. One can also see the apathy displayed by the rest of his family. Even though a member of the family has just died due to a tragic accident â€Å"Littlelessnothing!and that ended it (Frost 522)† they show no pain of the loss of a family member. It is depicted in the last two lines of the poem, â€Å"No more to build on there. And they, since they / Were not the one dead, turned to their affairs (Frost 522).† This shows that they had no emotion to the event, and went on to what they were doing as if nothing had happened in the first place. The second piece of poetry presented is one by James Langston Hughes. James Langston Hughes was born February 1, 1902 in Joplin Missouri. He spent his early life living with his grandmother in Illinois. Hughes began to write poems, and also some short stories, while he was in high school. Hughes mentions that the primary influences to his writing are Paul Lawrence Dunbar, Carl Sandburg, and Walt Whitman. His first book of poetry, entitled The Weary Blues, was published in 1926, while he was in college. Hughes graduated from Lincoln University three years following the publication of his first book of poetry. The year following his college graduated, Hughes  won the Harmon gold medal for literature for the first novel that he wrote, Not Without Laughter. James Langston Hughes poem â€Å"The Negro Speaks of Rivers† was the first poem of his that was published. This poem was also set to music later on. It is written from the perspective of a man that ties together African and African-American history. Hughes does this by naming different rivers that are in Africa and also those that are in the United States. This is where the wordplay of Langston Hughes can be seen. The type of pain that is displayed in this poem is not very obvious, but it is more implied than directly stated. Seeing that this poems speaks of African and African-American History, the idea of the oppression that these people groups have gone through is something that can be inferred from what the poem says. Both of these people groups have gone through major oppression because of slavery, inequality, and the like. (while it is not obvious I would recommend trying to find a few lines that can possibly show the pain) The final poem presented here is a poem from Emily Dickinson. Emily Dickinson was born in the year 1830 in a family that was considered to be very wealthy for that time period. Her father ultimately led the family and was a religious man for the family. He read prayers and passages of scripture to all that lived in the household to maintain this. She attended the seminary for a year, but went home after that year due to a significant amount of unpleasant experiences. After Emily left school, she isolated herself from all activities and responsibilities that were outside of the household, and kept to herself most of the time. She spent a significant amount of time reading books. Because of the morals that her father had, there were not many things for her to choose from, as her father thought that most books that were available at the time might shake up her thinking patterns. She then settled to read the Bible, classical myths, and also the works of William Shakespeare. Because of this, a great amount of the poems that she wrote had allusion to her readings contained in them. Although there is very little that people know of Emily Dickinsons outside life, but after reading the  poems that she has written, one can gain some access to the inside life in Emily Dickinson (Madden 1287). Emily Dickinson wrote nearly two thousand different poems in her lifetime (Madden 1288). Only but a few of these poems were intentionally published by her. Although Emily made her brother and sister promise to destroy all of her works following her death, her sister, Lavinia, could not gain the strength to destroy her sister Emily’s poetry. Not too far following her death in 1886, nine volumes of her works that were revised in wording, punctuation, structure, and rhyme were published. Unedited versions that were true to the original manuscript of Emily Dickinson where not published until 1955 (Madden 1288). Most of the poems of Emily Dickinson were her own personal laments that she did not intend for the public to ever see. â€Å"After A Great Pain, A Formal Feeling Comes† is an example of one of these extremely personal poems. During the time that this poem was written, Dickinson had just lost a very close friend. She was also beginning to dismiss the ideas of a career, starting a family, and making contact with anything or anyone that was outside of her own house. This whole poem directly deals with the pain of emotional loss that comes with the passing away of a person that is extremely close. Death was something that Dickinson never adjusted to, and it is displayed in this poem. She depicts how the feeling sits heavily and does not seem to go away very quickly â€Å"The Nerves sit ceremonious, like Tombs(Dickinson 1291)† (Lundin 95). In the last two lines of the first stanza Dickinson says, â€Å"The stiff Heart questions what it He, that bore, / And Yesterday, or Centuries before? (Dickinson 1291)† Here she is reliving past pains and grief that have occurred in her life before the death of her friend. She also relives past painful moments in her life in the second stanza â€Å"The Feet, mechanical, go round (Dickinson 1291)† (Grabher 217). In the last stanza, Dickinson focuses on the present pain that is in her life. â€Å"This is the Hour of Lead (Dickinson 1291)† refers to the passing of  Dickinson’s close friend. She then goes over the stages of how she moves on from these painful experiences: â€Å"As Freezing persons, recollect the Snow / FirstChillthen Stuporthen the letting go (Dickinson 1291)† The way that she ends this poems makes it appear as though she is trailing off into a land of thought to go dwell on what has just happened, to begin her process of recovery (Lundin 234). As one can see, many different aspects of pain have been discussed. Robert Frost’s â€Å"Out, Out† discussed physical pain due to an injury, and also the pain of stress due to that injury. James Langston Hughes implied the racial oppression of Africans and African-Americans that had gone before him in â€Å"The Negro Speaks of Rivers.† Emily Dickinson goes deep into her personal life and displays emotional pain with â€Å"After A Great Pain, A Formal Feeling Comes† by reminiscing on past grief and dealing with a new grief due to the death of a friend. As one reads through and analyzes these poems, one can see the way that pain is displayed in the midst of them and how each separate type affects people in different ways.

The weather is really bad, and it so dark even he has good eyes but he can’t see anything if it kind of far. The weather is an effect to how Rainsford fell in the ocean. Rainsford external conflict show that how he versus the nature and himself. Rainsford’s internal conflict created many mental challenges for him. For example, when he fell out of the ship, in the ocean he had to stop panicking or he would drown â€Å"A certain cool headedness had come to him it was not the first time he had been in a tight place†. He had been danger place many times so he had more experience and know what to do. He is a very brave person, if it was someone else may be the will be really scared and don’t know what to do. In addition, when in the jungle he keeps telling himself that he will not lose his nerve â€Å"I will not lose my nerve. I will not†. He had to control himself to go through all the game. If he being scared and don’t know do anything he may lose the game. In Rainsford internal conflict it show he is a brave person, how can he control his thinking to be life. Connell has written a story about hunted people, a serious game in the world. There is a lot of people get lost, have to play the game, and got killed by a man. Nobody knows who they are. But one day, a man comes to the island. He also has to play the it. â€Å"The Game†. And he win.

Patient Case Studies for Accurate Nursing Assessments Jon Teegardin Performing accurate nursing assessments on patients establishes a baseline of information regarding a patient’s complaint and history of present illness. The patients represented in this paper are not real, instead they are created to allow for examples of basic nursing assessments. This paper will assess two patients, one with chronic lower back pain and one with psychiatric and substance abuse complaints. Patient one The first patient is a 45 year old female who presents to the emergency room after arriving by private vehicle with a complaint of lower back pain with radiation down the right leg to the foot. My assessment of the patient begins when I go to the lobby and call the patients name. The patient ambulates with an even steady gait to the examination room. The patient does not require assistance ambulating. Once in the examination room, privacy is provided and an interview and full assessment are conducted. After introducing myself, the patient is identified using two identifiers, name and birthdate. The information provided by the patient is verified against the chart and an identifying wristband is applied. The chief complaint is then verified using an open ended question: Can you tell me why you are visiting the emergency room today? The patient’s response is â€Å"Back in 20012 I injured my back when my boyfriend was drunk and he slammed me against a wall. The doctor told me I had several ruptured discs in my back and it hurts so bad tonight I can’t stand it. The pain started about three days ago and has gotten progressively worse. I reassure the patient that she will be treated as soon as we can get the doctor in the room (Jarvis, 2012). The patient then asks for an emesis bag and states â€Å"I’m hurting so bad I think I’m going to throw up†. An emesis bag is provided. The patient retches several times and spits into the bag. No vomitus is noted . I continue with my interview by asking about the patients’ health history (Jarvis, 2012). I ask the patient about medication allergies. The patient states she is allergic to penicillin’s. Next I ask the patient about any medications she currently takes. The patient reports she takes the following medications: Xanax, 1 milligram 3 times per day, Gabapentin, 400 milligrams 3 times per day, Vicodin 10mg-325mg every 4 hours for pain, Lisinopril 20 milligrams, every day, Simvastatin 40 milligrams every day, Cyclobenzaprine, 20 milligrams every 6 hours, Ultram, 50 milligrams every 8 hours as needed for pain. When asked for a primary care physician, the patient states â€Å"I have recently switched doctors and I have an appointment on Wednesday with my new doctor. The patient presents an appointment card for a local physician. The patient is next asked about her medical history. The patient reports a history of hypertension, high cholesterol, anxiety, and ruptured discs in her lower back. The patient reports a surgical history of bilateral breast augmentation, bilateral tubal ligation, appendectomy, total hysterectomy, and tonsillectomy/adenoidectomy. When questioned about any surgical interventions for her back, the patient states â€Å"I don’t have insurance so I can’t afford to get any surgery on my back†. The patient reports a family history of hypertension, anxiety, and depression on her mother’s side. She also reports smoking 1 pack of cigarettes per day for 10 years but quit smoking 15 years ago. With my initial interview complete, the physical assessment begins (Jarvis, 2012). The patient is placed in a hospital gown for the examination. Vital signs are obtained. The blood pressure is 168/109 in the left arm, 166/106 in the right arm, heart rate is 79, oral temperature is 98.5, respirations are 16 per minute, oxygen saturation is 97% on room air, and the patient rates her pain on a numeric scale as a 10 out of 10 in her lower back. She describes the pain as a sharp, constant pain that is aggravated with movement, and helped with medication. She also relates radiation of the pain to her right leg as a burning sensation. The patient has shoulder length hair that is clean and well groomed. She denies any hearing problems, visual problems, congestion or cough. No drainage is noted from her ears, the eyes are clear with no redness or conjunctiva. Pupils are equal and reactive to light. Nares are clear bilaterally without swelling. The patient has good dentition with evidence of d ental intervention with fillings visible in three teeth. The lips, tongue, oral mucosa, and uvula are unremarkable. Facial symmetry is good with no drooping. The patient’s neck is supple with full range of motion and the trachea is midline. Respirations are clear and even bilaterally. The heart is auscultated and is strong and even at 78 beats per minute. Normal S1 and S2 are present. The patient denies any pain or mass in the breasts and reports that she self-examines monthly and her last mammogram was two years ago. Hand grips are strong and equal, radial pulses are strong and equal bilaterally. The abdomen is soft and non-tender to palpation. Bowel sounds are present in all four quadrants. A healed surgical scar is noted in the lower right quadrant. The patient denies any incontinence of bladder or bowel, and reports nausea and vomiting related to her back pain. The back is grossly unremarkable, and is tender to palpation in the area from L1 to L5. The patient denies any b urning or pain on urination, and denies flank pain. The patient reports a burning sensation down the right leg to the foot. Foot strength equal bilaterally, with strong bilateral pedal pulses. A scar is noted on the anterior right lower extremity distal to the patella. The patient reports that she accidently cut herself there as a child and required stitches to close the laceration. The patient’s mood and affect are appropriate for her age and the current situation. Her speech is clear. The patient is reassured that the physician will see her and the bed is verified to be in its lowest position with the wheels locked. The call light button is explained to the patient and placed within easy reach. The patient is reminded to request assistance prior to ambulating. Summary of findings My summary of finding is that the patient appears to be suffering from chronic back pain related to a traumatic injury in her past, and has uncontrolled hypertension related to noncompliance with her medication regimen. SOAP note S: Patient reports excruciating back pain radiating to the right leg, with nausea and vomiting prior to arrival. She also reports being out of her prescription medications which include a blood pressure medication. O: the patients’ blood pressure is elevated at 168/109, the lower back is tender to palpation. The patient ambulates without assistance but appears to be in mild distress. A: Nontoxic appearing white female that appears consistent with her stated age of 45 years old. EENT within normal limits. PERRLA at 3mm. Facial symmetry equal with no facial droop noted. The neck is supple and trachea is midline. Regular apical rate with S1 and S2. No S3 or S4 noted. Hand grips strong and equal bilaterally. Radial pulses strong and equal bilaterally. Respirations are even and unlabored. Lungs clear to auscultation bilaterally. The abdomen is soft and non-tender. Bowel sounds present in all four quadrants. Tender in the lower back region from L1 to L5 with radiation of pain to the right leg. No vomiting noted during assessment. Denies diarrhea. Denies urinary symptoms. Leg strength strong and equal bilaterally. Ambulates without difficulty or assistance. Pedal pulses present with no edema noted in lower extremities. P: Patient placed on non-invasive blood pressure monitoring and positioned for comfort. Expect MD orders for oral anti-hypertensive to reduce blood pressure and intramuscular injection of narcotic pain medication with antiemetic to control nausea and vomiting. Prepare patient for X-ray of lumbar spine region, and possible CT of the same area. A urine pregnancy test is contraindicated because of history of hysterectomy. Potential for admission to hospital for consult with orthopedist and surgical intervention. Patient two The second patient is a 62 year old female who has a chief complaint of alcohol dependence, benzodiazepine dependence, and depression. This patient is called from the lobby and ambulates to the exam room with an even, steady gait. Once in the examination room, privacy is provided and an interview and full assessment are conducted. I identify myself and the patient is identified using two identifiers and an identification band is placed on her wrist. I begin by obtaining a social history of the patient (Jarvis, 2012). She states she has never seen a psychiatrist and has been treated for depression by her primary healthcare provider. The patient is currently employed as a nurse practitioner/midwife with a county health department. She thoroughly enjoys her job and is fearful of losing her position due to chronic absenteeism related to her alcohol abuse. She reports consuming approximately one half gallon of liquor daily and has smoked one pack of cigarettes per day for twenty years. In her spare time she says that she enjoys sewing and gardening. The patient describes childhood as â€Å"normal†, had a â€Å"great relationship† with her father, but states â€Å"my mother loved my brother more than me, and nothing I did was good enough for her, even though I got straight a grades in school, and was active in church and band†. She further states â€Å"my mother used to beat me for waking up in the morning, becau se she said that I would eventually get into some kind of trouble†. †My brother is gay, but very successful working in Atlanta. I don’t see him enough, and I don’t hold what my mother did against him. I wish we saw more of each other. I got divorced over my drinking, so that was that, I could deal with all of it, up until my daughter died. Any other night I would have watched TV with her, but I was so tired that night. I went straight to bed and she decided to drive down to her brothers, 2 miles away. She made it about halfway, and was killed when she went off the road and hit a culvert, causing her car to roll several times. She died instantly†. Having obtained a social history, I begin obtaining medical history from the patient. The patient reports that she is allergic to codeine, Demerol, sulfa drugs, and Zithromax. She reports having a hysterectomy, cholecystectomy, and left foot ORIF. She is currently taking clonazepam, Lexapro, singular, and Xanax. She reports her mother and father had a history of alcohol abuse and are both deceased. My next assessment is a mental status examination. Having built a therapeutic relationship with the patient, I ask the patient if she has considered harming herself or had any command hallucinations (Jarvis, 2012). The patient denies any suicidal ideation or any hallucinations of any sort. The patient is well dressed, clean, pleasant, and cooperative. Her thought process is coherent with no ambivalence. The patients affect is calm and her appropriateness of mood to thought is normal. There is no depersonalization. The patient does not appear to be delusional, obsessive, or display ideas of reference. She is oriented to person, place, time, and situation. Vital signs are obtained and are all within normal limits. The patient denies any pain. Summary of finding My summary of finding is that this patient is suffering from depression, related to her divorce and the death of her daughter. Her needs include counseling, detox, and peer support. The patient’s education is an asset to her treatment. She has the support system of two sons. Stressors include her divorce due to her alcoholism and the sudden death of her daughter. Her coping methods include alcohol abuse, benzodiazepine abuse, and social isolation. SOAP note S: The patient reports feeling depressed and abusing alcohol and benzodiazepines. She denies suicidal ideation. O: The patient is well dressed, clean, pleasant, and cooperative. Her thought process is coherent with no ambivalence. The patients affect is calm and her appropriateness of mood to thought is normal. There is no depersonalization. The patient does not appear to be delusional, obsessive, or display ideas of reference. She is oriented to person, place, time, and situation. Vital signs are obtained and are all within normal limits. The patient denies any pain. A: The patient is suffering from depression related to her divorce and the death of her daughter. She could benefit from an inpatient rehab program. P: Prepare patient for lab draws to obtain baseline values, presence of drugs of abuse, and medical clearance. A call to the Georgia Crisis Access Line is anticipated for placement of the patient in an inpatient detoxification/rehabilitation program. Referral to grief counselor is also a possibility. Shakespearean Theater :: William Shakespeare

Shakespeare was fortunate to begin his career in the late 1500s, when English theater was going through major changes. Professional actors had been performing in England for centuries. Called "Players," they traveled from town to town, setting up makeshift stages in public halls, marketplaces, and the courtyards of inns. Often they met hostility from local authorities, who believed that crowds of playgoers were a magnet for crime and also contributed to the spread of disease. Actor James Burbage built England?s first permanent playhouse in 1576. Other open-air theaters sprang up during the next few decades. These playhouses were all located in the suburbs rather than in London, which had strict laws governing entertainment. In 1599 Shakespeare's built their own playhouse, the Globe. This roughly circular building had three levels of covered galleries. A platform stage about forty feet wide projected out into the open courtyard, where people who paid the lowest admission price could stand and watch the play. Admission to the gallery benches cost about twice as much. Wealthy people paid sixpence (what a skilled laborer earned in a day) to sit in the "lords' room," the part of the gallery directly over the stage. In all, Shakespeare's Globe could accommodate about 3,000 spectators. Toward the end of his career, his company acquired a fully enclosed theater in London for the winter season. The stage at the Globe had trapdoors for the entrance and exit of actors playing ghosts or other supernatural characters. At the back of the main stage was a small, curtained inner stage used for indoor scenes. Above this stood a two tiered gallery. The first tier was used to stage balcony and bedroom scenes, the second to house musicians. Sound effects, such as the booming of thunder, were produced in a hut on top of the stage roof. All performances took place in the afternoon because there was no artificial lighting. The stage was mostly bare. There were few props and no movable scenery. Instead of relying on scenery, Shakespeare used descriptive language to help audiences visualize the settings of his plays. For example, his long descriptions of the moon are more than just beautiful writing---they reminded Elizabethan audiences that the characters were meeting at night. What the Elizabethan stage lacked in scenery, it made up for in costumes. Shakespeare?s audiences considered clothing an important indication of social rank, so they demanded extravagant?if not always historically accurate---costuming. Along with its playbooks, an elaborate wardrobe was an Elizabethan theater company? Euthanasia In today's society there are many disagreements about the rights and wrongs of euthanasia. Although death is unavoidable for human beings, suffering before death is unbearable not only for terminal patients but for the family members and friends. Euthanasia comes from the Greek word "Thanatos" meaning death and the prefix "eu" meaning easy or good (Russell 94). Thus, "eu- Thanatos" meaning easy or good death. Euthanasia is a better choice for terminal patients than suicide. In our society, suicide is always traumatic for families and friends. If there is no alternative to relieve the suffering of terminal patients, then the more humane option to suicide is euthanasia. An option for people that are unsure of euthanasia is called DNR or do not resuscitate(McCuen 2). This means that if the patient has a heart attack or another potentially fatal problem in the hospital, the doctors are told to preform a " No Code" which means that they should let the patient die peacefully with any amount of painkillers or medication requested by the patient(McCuen 3). Doctors that are treating a dying patient should treat them with care. They should make them as comfortable as possible and give them medication to dull their pain. Instructors tell doctors in training to treat dying patients as people " that are alive but just that their death is more imminent than our own"(Moroney 2). Many families could rest easy that their relative is being treated well and are receiving the necessary attention. A lot of terminal patients decide to sign DNR forms or request euthanasia so that they would not be a burden on their families. Euthanasia should be legalized in all of the United States because it would end much suffering and pain. Euthanasia is the justified killing or assisted killing of a disabled or terminally ill person at their will or if the patient is incapable of making the decision, at the family's will(Satris 260). The doctor or in some cases, a family member, uses injections, weapons, or other means of killing to act out euthanasia. But euthanasia is usually gentle and quiet. In most of the cases of euthanasia, the patient died by lethal injection or certain life support systems were withheld.
